This testimony must be completed by an expert witness. The expert witness must only tick statements where the candidate has been observed carrying out duties during normal work activities/operations.

Coordinating preparation for site operations in the workplace Unit
Carry out site layout consultations to identify safe access points for the site and works, including works traffic and pedestrian segregation.
Agree and confirm safe access points that will maintain safety and minimise disruption.
Planning work activities and resources to meet project requirements in the workplace Unit
Inform stakeholders of updates that should be made on works programmes and schedules.
Implementing, maintaining and reviewing systems for health, safety, welfare, wellbeing and environmental protection in the workplace Unit

– health and safety requirements
– co-ordination of work procedures
Give appropriate advice and information to relevant people about the different methods of carrying out occupational work activities to achieve the required outcome.
Controlling work against agreed quality standards in the workplace Unit
Clearly record and communicate to the relevant individuals, their responsibilities in maintaining quality standards for the work or task.
Implement specified checks and record that work conforms to the agreed quality standards.
Identify variations in quality and their programme and safety implications.
Identify and report the proposed solutions to decision makers which will enable work to meet agreed quality standards, programme and safety requirements.
Supervising the installation, maintenance, monitoring and removal of temporary works in the workplace Unit
Observe current legislation and official guidance to the work environment to ensure protection and safety of the workplace, the general public, visitors and the environment by the application of information relating to:
– methods of work
– risk assessment
– safe use and storage of tools
Appraise the quality of the resources.
Apply project and organisational requirements to obtain a permit to load from the relevant responsible person as denoted by industry standards.
Check and confirm that all materials, equipment and tooling is correct prior to installation.
Supervise the removal of the temporary works in accordance with the programme of works to meet the needs of organisational and other stakeholder requirements.
