Witness Testimony
This testimony must be completed by an expert witness. The expert witness must only tick statements where the candidate has been observed carrying out duties during normal work activities/operations.
Definition of an Expert Witness
The Expert witness must know the candidate, have full familiarity of the work activities identified in the testimony, be in a supervisory or mentor position, and have enough thorough understanding of the item of plant and the application of the work that the testimony relates to.

Witness Testimony Criteria
Within the last 2-3 years, the above named at various stages during work has demonstrated that they:
Workplace Health, Safety and Welfare Unit
Has the ability to identify, record or report hazards within the workplace in accordance with organisational procedure
Has worn the appropriate PPE for the task being undertaken at all times in compliance with the company’s Health and Safety policy and safety signs
Has a knowledge of the safe use of fire extinguishers, appropriate to the type of fire
Complies with organisational procedures for maintaining the security of the workplace, during the working day, on completion of the day, from other personnel and theft
Complies with the organisational policy for emergency procedures in case of accidents and/or fire
Productive Work Practices Unit
Is capable of efficient communication with their work colleagues, operatives from other trades and their supervisor/client
Understands the importance of reporting any problems with the progress of the work to their supervisor
Is able to offer ideas in overcoming problems within their own occupational area and relaying information regarding programme changes to work colleagues
Has the ability to coordinate with other trades that may be affected by their work
Assists in monitoring the progress of the work by checking that they have all the information they require with regard to materials, labour and plant in order to carry out the work
Completes timesheets or other records as required, relating to the work carried out, the hours worked and plant and materials used
Operate and control operations of road plant, machinery or equipment Units
Can interpret and confirm the relevance of given information including manufacturers instructions, drawings and specifications relating to the preparation and operation of plant or machinery for use
Can organise and communicate with others the sequence of how the work is to be carried out when preparing for use and operating plant or machinery
Complies with current regulations, legislation and company policy and uses the correct PPE when preparing for use or operating plant or machinery
Has requested resources, materials and consumables following company procedures when preparing for use or operating plant or machinery
Has selected plant resources, attachments, tools and equipment appropriate for intended work operations when preparing for use or operating plant or machinery
Protects the work and surrounding areas, minimises damage and disposes of waste following company procedures when preparing for use or operating plant or machinery
Complies with given (contract) information in order to carry out work effectively whilst preparing for use or operating plant or machinery
Has fitted, attached, set up, adjusted, secured, checked, confirmed and removed items/ parts/ components and/or attachments of the plant or machinery they have used
Uses and maintains appropriate hand tools and accessories whilst preparing for and during work
Is capable of preparing plant or machinery (for operational use) to given working instructions by setting up and carrying out functional checks, checking operational performance and ensuring safety and security
